Personal Assistant (PA) Transition Process

Getting started is easy! Call Ideal Home Health at 347-315-2402 to select us as your facilitator, or submit your information through our online form. From there, we’ll guide both personal assistants and consumers through the registration process to ensure a smooth transition to CDPAP. 

Once your information has been received, we’ll reach out directly to get the process started!

Registration Process:

Complete Required Documents in PPL@Home

Log in to PPL@Home and complete the required documents:

  • Unexpired I-9 Documents.
  • IRS Form W-4 (Federal tax withholding form).
  • New York State Income Tax Form IT-2104 (State tax withholding form).
  • Direct Deposit Form (optional but recommended for payroll).
  • PA-Consumer Agreement (outlines your role and responsibilities).
  • Health Assessment Attestation (if applicable, for a delay in submission of health records).
  • Physical Form on File

Complete Mandatory Training

Access the training section in PPL@Home and complete the required courses:

  • Electronic Visit Verification (EVV) training.
  • Compliance and Program Orientation.
  • Infection Control and PPE.
  • HIPAA (Privacy Regulations).
  • Elderly Abuse and Neglect.
  • Diversity and Inclusion (Cultural Competency).

Prepare for Payroll

  • Set up your direct deposit and ensure all forms are completed.
  • Once all documentation and training are completed, you will be notified when you can begin submitting timesheets through PPL’s Time4Care app.
