Consumer Transition Process

Getting started is easy! Call Ideal Home Health at 347-315-2402 to select us as your facilitator, or submit your information through our online form. From there, we’ll guide you through the registration process to ensure a smooth transition to CDPAP.

Once your information has been received, we’ll reach out directly to get the process started!


Registration Process:

Complete Required Documents in PPL@Home

Log in to PPL@Home and complete the required documents:

  • Consumer Memorandum of Understanding (MOU) – Outlines your responsibilities as a joint employer under CDPAP.
  • Consumer-PA Agreement – Defines the relationship between you and your Personal Assistant (PA).
  • Designated Representative Form (if applicable) – Only required if you have a representative managing your care.
  • Legal Guardian/Power of Attorney (POA) Documentation (if applicable).

Support Your PA’s Registration

To ensure a smooth process, help your PA (Personal Assistant) complete their registration:

  • Confirm your PA has received their registration link and encourage them to log in to PPL@Home.
  • Assist with uploading any required documents for your PA, such as health assessments or immunization records.
  • Prepare for Orientation and EVV.


Once registration is complete, you’ll attend an orientation session with your facilitator, where you’ll learn about:

  • Service Authorizations – Understanding your approved care hours.
  • Electronic Visit Verification (EVV) System – Required for logging PA work hours.
  • Timesheet Submissions & Payroll – Ensuring your PA is paid accurately and on time.
