Consumer Transition Process

Submit Your Information to Ideal Home Health

Fill out the form on this page to provide your contact details and confirm the consumer you work with.
Once your information has been received, you can expect an email from PPL with a link to the PPL@Home registration portal.

Complete Required Documents in PPL@Home

Log in to PPL@Home and fill out the following forms:
Consumer Memorandum of Understanding (MOU): Outlines your responsibilities as a joint employer under CDPAP.
Consumer-PA Agreement: Defines the relationship between you and your personal assistant.
Designated Representative Form: only if you have a representative assisting with managing your care.
Legal Guardian/Power of Attorney (POA) documentation (if applicable).

Support Your PA’s Registration

Confirm that your PA has received their registration link and encourage them to complete their paperwork in PPL@Home.
Assist with uploading any required documents for your PA, such as health assessments or immunization records.

Prepare for Orientation and EVV

Attend a scheduled orientation session with your facilitator to learn about:
Service Authorizations.
Electronic Visit Verification (EVV) system training.
Timesheet submissions and PA payroll.
