
Everything You Need to Register and Stay Compliant with NY CDPAP
The Consumer Directed Personal Assistance Program (CDPAP) empowers you to choose and direct your own caregiver including family members and friends. This guide will walk you through the entire process, from initial registration to ongoing compliance requirements.

The Consumer Directed Personal Assistance Program (CDPAP) is a New York State Medicaid program that allows you to:
Must be 18 years or older
Active NY Medicaid coverage
Enrolled in Medicaid Managed Care or MLTC plan (in most cases)
Need help with Activities of Daily Living (ADLs)
Examples: bathing, dressing, eating, mobility
Able to direct your own care
OR have a Designated Representative who can
ADLs include: bathing, dressing, eating, toileting, transferring (moving from bed to chair), continence care, and ambulation (walking/mobility).
Ensure you have active New York State Medicaid coverage. If you're in NYC or Nassau County, you'll need to enroll in a Medicaid Managed Care plan. Dual eligible consumers (Medicare + Medicaid) must enroll in a Managed Long-Term Care (MLTC) plan.
Reach out to your insurance plan or local DSS to request CDPAP services. They will initiate your application and assessment.
A nurse from your MLTC plan will visit to evaluate your needs and determine your approved care hours. This assessment measures your level of need for home care services.

A login will be created for you in the PPL portal, and we will reach out to assist with registration.
Choose your caregiver and help them complete their registration. Your PA must complete all onboarding requirements before they can begin providing care.
Once everything is approved and your PA is registered, care can begin! You'll direct your PA's schedule and tasks according to your care plan.
Bring your active NY Medicaid card or documentation showing current coverage
Valid ID such as Driver's License, NY State ID, or Passport (must be unexpired)
Required for payroll and tax purposes
Current phone number, email address, and mailing address
Name of your MCO or MLTC plan and member ID number
Approved hours of care from your insurance plan (obtained after UAS-NY assessment)
Legal guardianship papers or healthcare proxy if you have a DR directing your care
Indicate your preferred language for communication
All documents must be original or certified copies - photocopies will not be accepted
IDs must be unexpired - expired documents cannot be processed
Once you're enrolled, staying compliant ensures uninterrupted care and proper payment for your PA. Here's what you need to do:
Review and approve your PA's hours every week. Timesheets not approved by noon Sunday will delay payment.
Your PA should clock in/out using the Time4Care app or PPL's approved EVV system for every visit.
Your PA should maintain at least 90% EVV compliance.
Notify your MCO and Ideal CDPAP at (347) 315-2402 if your care needs change or your authorized hours need adjustment.
Ensure your PA completed required training in the Nevvon portal.
Complete annual UAS-NY reassessments to maintain your approved care hours.
Notify Ideal CDPAP at (347) 315-2402 and PPL at 1-833-247-5346 immediately if your phone number, address, or email changes.
Keep your Medicaid active by completing annual renewals with your local DSS.
If you switch insurance plans, notify Ideal CDPAP at (347) 315-2402 immediately. If your MCO status shows "Undetermined," contact your insurance and have them send updated authorization to PPL.
If you hire a new PA, complete their registration as soon as possible to avoid gaps in care.
