About Ideal CDPAP

Ideal CDPAP is an approved facilitator for New York's Consumer Directed Personal Assistance Program (CDPAP). We partner with Public Partnerships LLC (PPL), the statewide fiscal intermediary, to ensure you receive quality care from your trusted personal assistants.

How to Get Started

👤 For Consumers

Call Ideal CDPAP at (347) 315-2402 to select us as your facilitator (have your Medicaid ID ready)
Complete the MOU form online via PPL@Home or in-person at our office
Work with your PAs to complete required forms and training

Call Ideal CDPAP with your Consumer at (347) 315-2402 (Consumer's Medicaid ID required)
Complete required forms online via PPL@Home or in-person
Complete mandatory PPL training modules
