What is CDPAP?

The Consumer Directed Personal Assistance Program (CDPAP) is a New York State Medicaid personal care aide program that allows you to:

  • Choose your own caregiver (called a “Personal Assistant” (or PA)
  • Hire family members or friends (except spouses, parents of consumers under 21, or your designated representative)
  • Direct your own care and schedule
  • Have your PA paid directly through Medicaid

Initial Registration Process

Verify Medicaid Eligibility

Ensure you have active New York State Medicaid coverage. If you’re in NYC or Nassau County, you’ll need to enroll in a Medicaid Managed Care plan. Dual eligible consumers (Medicare + Medicaid) must enroll in a Managed Long-Term Care (MLTC) plan.

Contact Your Managed Care Organization (MCO) or Local Department of Social Services

Reach out to your insurance plan or local DSS to request CDPAP services. They will initiate your application and assessment.

Complete UAS-NY Assessment

A nurse from your MLTC plan will visit to evaluate your needs and determine your approved care hours. This assessment measures your level of need for home care services.

Choose Ideal CDPAP as Your Facilitator

Select Ideal CDPAP to handle your registration, compliance, and administrative needs. You can register directly on our website at IdealCDPAP.com or call us at (347) 315-2402.

Complete Registration with PPL

A login will be created for you in the PPL portal, and we will reach out to assist with registration.

Select and Register Your Personal Assistant (PA)

Choose your caregiver and help them complete their registration. Your PA must complete all onboarding requirements before they can begin providing care.

Begin Receiving Care

Once everything is approved and your PA is registered, care can begin! You’ll direct your PA’s schedule and tasks according to your care plan.

Initial Registration Checklist

Consumer Registration Documents

  • Medicaid Card or Proof of Coverage: Bring your active NY Medicaid card or documentation showing current coverage
  • Government-Issued Photo ID: Valid ID such as Driver’s License, NY State ID, or Passport (must be unexpired)
  • Social Security Card or Number: Required for payroll and tax purposes
  • Contact Information: Current phone number, email address, and mailing address
  • Managed Care Plan Information: Name of your MCO or MLTC plan and member ID number
  • Care Plan or Authorization: Approved hours of care from your insurance plan (obtained after UAS-NY assessment)
  • Designated Representative Documentation (if applicable): Legal guardianship papers or healthcare proxy if you have a DR directing your care
  • Primary Language Preference: Indicate your preferred language for communication

Ongoing Compliance Requirements

Once you’re enrolled, staying compliant ensures uninterrupted care and proper payment for your PA. Here’s what you need to do:

PA Training Deadline

All Personal Assistants must complete training in the Nevvon portal. Failure to complete training may result in non-compliance and affect your PA’s ability to continue working.

Weekly Schedule to Remember
  • By Saturday 11:59 PM ET:
    PAs must submit all time entries

  • By Sunday 12:00 PM (Noon) ET:
    Approved hours are processed for payment after Sunday approval

PPL’s approved EVV system for every visit.

Monthly Responsibilities
Review EVV Compliance Rate

Your PA should maintain at least 90% EVV compliance.

Update Any Changes in Care Needs

Notify your MCO and Ideal CDPAP at (347) 315-2402 if your care needs change or your authorized hours need adjustment.

Verify PA Training Completion

Ensure your PA completed required training in the Nevvon portal.

Annual/As-Needed Responsibilities
Participate in Re-Assessment

Complete annual UAS-NY reassessments to maintain your approved care hours.

Update Contact Information

Notify Ideal CDPAP at (347) 315-2402 and PPL at 1-833-247-5346 immediately if your phone number, address, or email changes.

Renew Medicaid Coverage

Keep your Medicaid active by completing annual renewals with your local DSS.

Report MCO Changes

If you switch insurance plans, notify Ideal CDPAP at (347) 315-2402 immediately. If your MCO status shows “Undetermined,” contact your insurance and have them
send updated authorization to PPL.

Register New PAs Promptly

If you hire a new PA, complete their registration as soon as possible to avoid gaps in care.

How do you enroll in CDPAP in New York?

Who is eligible for CDPAP in NY?

To be eligible for CDPAP in New York, you must be enrolled in New York State Medicaid or a Medicaid managed care plan, have a stable medical condition that requires hands-on personal assistance services, be able to direct your own care or have a designated representative who can do so on your behalf, and have a physician or other qualified healthcare professional order CDPAP services as part of your care plan. There is no age restriction — CDPAP is available to adults and children who meet the Medicaid and medical criteria. What are the requirements for CDPAP caregivers in New York?

A CDPAP personal assistant must be at least 18 years old, legally authorized to work in the United States, and able to pass the required background checks and clearances. They cannot be the consumer’s designated representative or legally responsible relative — but they can be an adult child, sibling, parent of an adult consumer, or a friend. No prior medical training or certification is required; the consumer directs all care. PAs must complete registration through PPL, finish required training, and set up the Time4Care app for electronic timekeeping. Ideal CDPAP walks every PA through this process at enrollment and supports them throughout.

Can I enroll in CDPAP if I already have a home care agency?

Yes. If you’re currently receiving home care through a traditional agency but would prefer to choose your own caregiver — including a family member — you can transition to CDPAP. The process involves selecting a PPL facilitator like Ideal CDPAP, completing the MOU, and having your physician update your plan of care. Your Medicaid coverage and personal assistance services remain continuous throughout the transition. We handle the coordination so there’s no gap in care.

What is a designated representative, and do I need one?

A designated representative is someone who assists the consumer in directing their care when the consumer is unable to do so independently due to age or a medical or cognitive condition. This can be a family member, friend, or legal guardian. If you are fully able to direct your own care, you do not need a designated representative. If you do need one, they play a key role in the CDPAP enrollment process — selecting the PA, completing the MOU, and managing ongoing responsibilities. Note that the designated representative cannot also serve as the paid personal assistant.

What social services does CDPAP cover?

CDPAP covers personal assistance services — hands-on help with activities of daily living including bathing, grooming, dressing, toileting, mobility, meal preparation, light housekeeping, medication reminders, and transportation. It does not cover skilled medical services like wound care or IV therapy — those require a separate skilled nursing program. CDPAP is a Medicaid-funded program administered through New York State’s social services infrastructure, and all approved services must be documented in a physician-ordered plan of care.

What happens if my personal assistant can no longer provide care?

If your PA leaves, you have the right to identify a new one. You remain enrolled as a CDPAP consumer — your eligibility and approved hours are not affected by a change in PA. Ideal CDPAP will support you through the process of onboarding a new personal assistant, including the required PPL registration and training steps. We recommend having a backup plan in place before a transition becomes urgent, and our team can help you think through that proactively.
